1. INTRODUCTION

1.1 The videos and other content that you are about to access and view (the "Content") are brought to you by Nanyang Technological University through Lee Kong Chian School of Medicine's Centre for Population Health Sciences ("NTU", "our", "we" or "us"). Please read these Terms and Conditions ("Terms", "Terms and Conditions") carefully, and also YouTube's Terms of Service before viewing the Content (collectively referred to as the "Platform") as they set out the basis on which you are allowed to use the Platform.

1.2 By using or accessing the Platform, you agree that you have read and understood and agree to be bound by these Terms (and any other terms that we may make available from time to time). These Terms expressly supersede prior agreements or arrangements with you regarding the use of the Platform and constitute a legally binding agreement between you and us. If you do not agree to these Terms, please do not use or continue using the Platform and the Content. 1.3 Your access to and use of the Platform is conditioned on your acceptance of and compliance with these Terms. These Terms apply to all users who access or use the Platform. By using the Platform, you agree to the collection, use and disclosure of your Personal Data and Other Data in accordance with these Terms and the NTU Privacy Statement.

1.4 The Platform is not directed at any person in any jurisdiction where the publication or availability of them is prohibited. You must not access the Platform and the Content from any such jurisdiction. You are responsible for all compliance with local laws and regulations which apply to you.

2. DEFINITIONS

"Personal Data" has the same meaning as defined in the Singapore Personal Data Protection Act.

"Users" means users of the Platform, including you and "User" means any one of them.

3. ABOUT THE CONTENT

3.1 The Platform provides comprehensive footcare education for people living with diabetes and their caregivers through QR codes with hyperlinks to unlisted youtube videos developed by Nanyang Technological University.

3.2 The Platform offers 54 animations in four languages (English, Mandarin, Malay and Tamil) with a deep fake presenter covering evidence-based and expert-reviewed content in 21 units under 10 modules and 4 themes: introduction, inspection, prevention, and wound care.

4. ACCESS TO CONTENT

4.1 Instructions on accessing the platform

a) By accessing the Platform, you shall be deemed to have agreed to the terms and conditions included in the Google form and have consented to our collection, use, processing and transfer of all required data

b) After submitting the form you will see a "thank-you screen" with a link to a pdf file with a list of topics, QR codes and hyperlinks to videos in 4 languages.

c) Save the file to make it easily accessible for you. Next time you wish your patient to learn more about a specific topic:

- click on the selected hyperlink and hand the device you are using to the patient to watch the video. This will be useful if you have an access to a company tablet with the access to the internet. Or,

- show a selected QR code and ask the patient to scan it. After doing so, they can watch the video on their device.

4.2 Limitations to access.

a) In order to control the access to the materials developed by NTU, we kindly request you not to send (or allow to copy/take a photo of) the pdf file with/by the patient.

b) If you wish your colleague to get the opportunity to also benefit from the materials, please share the registration link where they can agree to the terms and conditions.

5. PERSONAL DATA

5.1 Log Data

(a) Like many content owners, we, through Youtube, collect aggregated information that your browser or device sends whenever you visit or access the content stored on Youtube ("Log Data").

(b) You agree and consent that we may use the Log Data (including but not limited to your personal data) for the purposes of notifying you about changes to the Platform, the Content and/or other related issues.

5.2 Use of Cookies

(a) Cookies are files with small amount of data, which may include an anonymous unique identifier. Cookies are sent to your browser from a web site and stored on your computer's hard drive.

(b) Like many sites, YouTube where the content is stored, may use "cookies" to collect information. You can instruct your browser to refuse all cookies or to indicate when a cookie is being sent. However, if you do not accept cookies, you may not be able to use some portions of the Platform.

5.4 How we use your Personal Data

We may collect, use, disclose and process your Personal Data for one or more of the following purposes:

(a) to provide the Content to you

(b) to manage the Content we provide (for eg. to see the statistics of views);

(c) any other purpose reasonably related to the aforesaid.

(collectively, the "Purposes").

5.5 Disclosure to Third Parties

(a) In the unlikely circumstance where we may need to disclose your Personal Data and/or Other Data to third parties, whether located within or outside Singapore, for one or more of the above Purposes, as such third parties would be processing your Personal Data and/or Other Data on our behalf for one or more of the above Purposes.

5.6 Security

(a) The security of your Personal Data is important to us but no method of transmission over the internet, or method of electronic storage, is 100% secure. While we strive to use commercially acceptable means to protect your Personal Data, we cannot guarantee its absolute security.

(b) Your Personal Data is stored at a secure server with multilayered encryption algorithms.

(c) By using the Platform and/or Content, you acknowledge and agree that the transmission of information via the internet is not completely secure and we cannot guarantee the security of your personal data transmitted through any online means. Therefore, you are strongly advised to take safety precautions and any transmission remains at your own risk.

6. INFORMATION PROVIDED BY PLATFORM

6.1 The Platform aims to provide helpful educational information to you and your patients in relation to you foot care. This includes information relating to physical, mental and emotional wellbeing (for example, types of issues that may be spotted on feet during inspection).

6.2 You acknowledge and agree that all information and materials provided by us and/or third parties via the Platform and Content, including in relation to footcare and diabetes self-management is provided for general information purposes only. Such information provided is not intended to be a substitute for professional medical consultation, advice or treatment from qualified doctors, nor should they constitute any health or medical advice.

6.3 Although we consulted the Content with third party professionals (such as digital health researchers, health care professionals) when curating the content on the Platform and Content, this content is not intended to diagnose, treat or cure any illness, condition or other physical or mental health issue. It is not intended to be, and should not be used in the place of, the advice of medical professionals or any information provided on any product packaging or label. For advice on medical, health and wellbeing issues you should always consult your doctor or other qualified health care professional. While the Content is created via on a synthesis of international healthcare literature and reviewed by a panel of healthcare professionals, they are for reference or illustrative purposes only. Hence, you should still seek professional or specialist medical advice before taking , or refraining from, any action on the basis of information or content made available via the Platform or Content.
